What it says

Ray - ALLERGIES. Written down the morning after he came home, because the hospital asked three times and he gave three slightly different answers. 1. Penicillin - hives all over his chest, 1986, after a strep throat. Amoxicillin counts. 2. Sulfa drugs (it was Bactrim) - rash and fever, 2003. 3. Adhesive tape - blisters. Ask for paper tape. No food allergies. NOT allergic to the contrast dye; he had it for the stent and was fine. Me, for completeness: no drug allergies. Latex gloves make my hands itch, and I tell the dentist every time.
