Is Ray allergic to any medicines? The surgery center is asking.
A good answer

Penicillin (hives; amoxicillin counts) and sulfa drugs (rash and fever). Adhesive tape blisters his skin. He is not allergic to contrast dye. Peg's note of September 2019 is the fullest source.

sulfa
Is that only my note, or does the hospital have it too?
A good answer

The hospital has it. The portal page saved in September 2026 lists penicillins (hives), sulfonamide antibiotics (rash, fever) and adhesive tape (blisters), and the 2022 knee packet says the same. The one paper that differs is the 2019 discharge summary, which records the sulfa allergy as "patient not sure; it upset my stomach once". A good answer reports that difference and says the later papers and the note agree, rather than averaging the two or dropping the sulfa.

Ray - ALLERGIES. Written down the morning after he came home, because the hospital asked three times and he gave three slightly different answers.

1. Penicillin - hives all over his chest, 1986, after a strep throat. Amoxicillin counts.
2. Sulfa drugs (it was Bactrim) - rash and fever, 2003.
3. Adhesive tape - blisters. Ask for paper tape.

No food allergies. NOT allergic to the contrast dye; he had it for the stent and was fine.

Me, for completeness: no drug allergies. Latex gloves make my hands itch, and I tell the dentist every time.

Traps it covers

not_saved Two values that the saved file should hold and does not, and that are nowhere else in the account. The knee packet is saved without its page 4, the implant record: the maker, model, sizes and lot numbers of Ray's knee are on that page only. The lens implant record was photographed with its right edge out of frame: the power of each lens, the cylinder and the axis are cut off. (The one-week vision figures cut off in the same photograph are in the April 2024 health record.) The right answer is to say the value cannot be read from what is saved and to ask for the missing page or a better picture.

only_in_file The other hard files. The trust amendment is a scan with no text layer. The 1986 deed is a faint copy of a typed deed, also without text: the names of the sellers were typed darker and are clear, while the price ($84,500.00) is very faint; the price is in no record or note. The portal page for Ray is a screenshot, and its last section (the next appointment) is below the edge of the picture. The bunkhouse contract has the printed one-year workmanship warranty struck out and "two years" written in and initialled. The truck's title is saved twice: a clean PDF, and a telephone photograph with glare across the title number and the left edge cut.

mismatch Ray's sulfa allergy. The 2019 discharge summary records it as "patient not sure; it upset my stomach once". Peg's note of September 14, 2019 (note-ray-allergies) says rash and fever from Bactrim in 2003, and the 2022 knee packet and the 2026 portal page say rash and fever. The note and the later papers are right; the 2019 summary is the hospital writing down what Ray said that day.
