What it says

RAY: HOME FROM THE HOSPITAL. What the cardiology nurse told us at discharge this noon, with the sheet in front of me. WHAT WAS DONE (Thursday, September 12, Dr. Valdivia): one stent, in the artery down the front of the heart. The other arteries have mild disease and need nothing now. The heart muscle is pumping normally. THE PILLS. Four are new. Ellen has them in the box with fourteen doors, morning and evening. Morning: aspirin 81 mg; clopidogrel 75 mg; metoprolol 25 mg (the long-acting kind; do not crush); lisinopril 10 mg, as before; metformin 1000 mg with breakfast, as before. Evening: metformin 1000 mg with supper; atorvastatin 40 mg at bedtime. N.B. His atorvastatin is the 40. Mine is the 20. The bottles are alike. Mine now has a red rubber band on it. THE ONE THAT MATTERS MOST: the clopidogrel, every day, for a year, with the aspirin. He does not skip it and does not stop it for a dentist or anyone else without Dr. Valdivia saying so. The aspirin is for good. The metoprolol is not to be stopped suddenly either. THE WRIST (the right one, where they went in): keep the dressing on until tomorrow. A bruise is expected. If it bleeds, press hard for ten minutes and call. If the hand goes cold, numb or blue, emergency room. WHAT HE MAY NOT DO: - Drive: one week. - Lift more than ten pounds: two weeks. That includes the dog, the rake, the outboard and the grandchildren. - Shovel snow: not this winter. After that, ask Dr. Valdivia. WHAT HE MUST DO: walk. Five to ten minutes, twice a day, flat ground, starting tomorrow, adding a little daily. Cardiac rehabilitation starts Tuesday, October 1, at 8: three mornings a week for twelve weeks, 36 sessions. CALL 911, not the clinic: chest pressure or pain that lasts more than a few minutes, pain going into the arm or jaw, shortness of breath at rest, fainting. CALL THE HEART CENTER (the number is on the refrigerator and in my telephone): bleeding that will not stop, black stools, a rash, dizziness on standing. THE CARD: a wallet card with the make and size of the stent and the date. He carries it. FOOD: what the diabetes class said, and less salt. No more summer sausage. Asked by Ray: may he go up north? After the 26th, if she says so. May he fish? Sitting down. May he run the table saw? Not this week. APPOINTMENTS: Dr. Valdivia, Thursday, September 26, 2:40. Dr. Raghavan in two weeks; I will telephone Monday.
