Question

What do I write for allergies and medications for each kid?
A good answer

Nora: amoxicillin (no penicillins), as her chart has had it since 2017; albuterol inhaler, 2 puffs before exercise or as needed. Eli: peanut; EpiPen Jr 0.15 mg as needed. Ivy: no allergies, no medications. A good answer may add that at Nora's August 2026 visit the pediatrician doubted the amoxicillin allergy, but it has not been tested, so it still goes on the form.

Traps it covers

mismatch Eli's allergy is in many places in different words. The note of October 4, 2023 is the fullest: peanut, not tree nuts, Ivy clear. The scribble from the appointment says "PN 14.6", the visit record says positive for peanut and everything else negative, the prescription says "for the peanut allergy", the action plan says PEANUT, the ER-night note names the peanut butter frosting. The school portal page says "Peanuts, tree nuts". The allergist's finding is right: peanut only. The school's entry is broader than the diagnosis.

mismatch Whether Nora is allergic to amoxicillin. Her chart has said so since a rash in 2017, and the immunization record and this year's inhaler form repeat it. In August 2026 the pediatrician said the rash may have been the virus and offered testing; nothing was booked. Nothing settles it. Forms should keep listing it.
