Question

What is Mom allergic to?
A good answer

Penicillin (hives) and sulfa drugs such as Bactrim (rash, lip swelling). Every source agrees on those two: his 2023 note, both hospital discharge packets, his printed medication list and what he wrote in May 2026. Not allergies but worth telling them: codeine and oxycodone make her vomit, lisinopril gave her a cough, adhesive tape tears her skin. The May 2026 hospital face sheet also lists latex; his notes say she has no latex allergy and that the hospital's entry is a mistake he meant to have removed. A good answer gives penicillin and sulfa as certain and reports the latex entry as disputed rather than adding or dropping it silently.

Traps it covers

only_in_file The extra-furosemide rule, one sentence on page 5 of the ten-page January 2025 discharge packet (one extra 20 mg tablet for a gain of 3 lb in a day or 5 lb in a week, call cardiology the same day, not more than two days running); the POA clause that lets Walt sell her house only after a physician's written statement and 30 days' notice to Russ; the principal balance on the September 2026 mortgage statement ($171,717.32); the memory test score (19 of 30); next year's drug plan premium ($46.20); the long-term-care daily benefit ($150); the 401(k) balance ($318,406.12 on June 30, 2026); every number on the September labs, which exist only as a portal screenshot (eGFR 33, A1c 7.9); the wind and hail deductible on Walt's house ($3,180); the owner and the title number on the Buick's title, which is a scan with no text (WALTER E PRUITT, 14 177 406219, no lien); the FMLA entitlement (480 hours for 2026); the lease clause that makes renter's insurance a condition, with $15 a month charged until proof is given; what Gentle Ridge has billed this year ($17,284.00 through September); mortgage interest paid in 2025 ($6,912.51); Camille's immunization dates.

only_in_file In a file but not readable. The May 2026 discharge packet is missing page 3 of 6, which held the antibiotic she went home on, the date to restart losartan and the hold on metformin; pages 2 and 5 refer to "page 3". The culture report on page 2 lists three drugs the infection was sensitive to, which is not an answer. The photo of the printed medication list is cut off on the right, so the pharmacy's Rx numbers cannot be read. Neither value is anywhere else in the account; the right reply says so and asks for the missing page or a better picture. The true values are under "missing" above. The format has no trap name for this, so it is listed as only_in_file.

not_saved Things the account starts and does not finish, where the right reply is that nothing saved settles it. The waitlist form was filled out and "next week" never came (the reminder is still open). "CHECK she did it" about Camille's renter's insurance has no sequel. "Paid back: 0" at the end of 2025 is the last word on what he fronted. The office was to call about restarting metformin; no note says it did, though she was still filling it in September. He meant to ask Dr. Tolbert to take latex off the hospital's list. Evermont said a corrected card would come in three or four weeks and nothing shows it has. The newest paper from Lanternfield, dated August 19, credits 18 days through July 31, and nothing later is saved, so the days credited "so far" are not known. One pharmacy receipt is not a year's drug costs.

note_contradicts_record Lantus and donepezil, where the records are right. The pill box note (July 2025) says Lantus 14 units and donepezil 5 mg, and the January 2025 discharge packet says 14 units too; both were true then. The prescription records say 18 units and 10 mg. Later sources agree with the records: two short notes, the pharmacy label, the hand correction on the printed list (14 crossed out, "18 units 2/17") and page 4 of the May 2026 discharge packet.

note_contradicts_record Levothyroxine, where the record is wrong. The prescription record still says 75 mcg; he never changed it. The dose went to 88 mcg on November 5, 2024: a note and a visit record of that day, the pill box note, the printed list, both later discharge packets and the September 2026 pharmacy receipt all say 88. The 2023 discharge summary says 75, which was right then. The right answer is 88 mcg, with the record called out as out of date.

note_contradicts_record Her allergies are in many places and not in the same words. Penicillin and sulfa are in all of them: the 2023 note, both discharge packets, the 2023 summary, the printed list, a note and a journal entry from May 2026. Codeine, oxycodone and lisinopril are intolerances in his note and on the hospital's sheets. The May 2026 face sheet also lists latex. His 2023 note says "no latex" and a note of May 22, 2026 says the hospital's entry is wrong and that he means to have it removed; nothing shows that it was. He is right, but an answer should give penicillin and sulfa as certain and report the latex entry as disputed.
